For All Other Excavation Permits:

1. All fees shall be paid prior to issuance of the permit, in accordance with the terms specified in Municipal Code Section 12.04.030.� If when completing the work, a utility cut is made which exceeds the size specified in this permit, the Contractor will remit to the City of Huntingburg any additional charge based upon the schedule of prices.
2. The Contractor shall notify the Indiana Underground Plant Protection Service (�Holey Moley�) at 800-382-5544 at least two (2) working days prior to the start of the work.
3. Not more than two (2) working days prior to the start of work, the contractor shall notify the city�s Office Manager at 812-683-2211 of the scheduled start date and time, the expected duration of work, and whether any streets or sidewalks will be partially or completely closed to traffic.� If delayed, this information will need to be relayed again to the Office Manager.
4. The work in this permit shall be conducted only during daylight hours.
5. Where any public street or sidewalk is partially or completely closed to pedestrian or auto traffic, traffic shall be prevented from entering the site or shall be routed through the site by barricades and lights furnished by the Contractor, and as specified and required by the Street Superintendent.
6. The Contractor shall be responsible for backfilling all utility cuts, under the supervision of and to the satisfaction of the Street Superintendent.� Following acceptance of the Contractor�s work, the Street Superintendent shall be responsible for maintaining the excavation area and for performing all repaving work necessary to return the disturbed area to its original condition.
7. The Contractor shall be responsible for restoring all sidewalks and curbs to the standards of the City of Huntingburg.

I certify that this application is true and correct, and I agree to indemnify and hold harmless the City of Huntingburg and its employees for any loss, liability or damage that may result or accrue from or because of the applicant�s negligence in performing the work under this permit, or providing the necessary barriers and warning devices, or disregarding any legitimate order of the Street Superintendent or his agents relating to any excavation or tunnel.
